PARTICIPANT WAIVER AND RELEASE OF LIABILITY

Greenbrier Mountain Festival – Special Activity Waiver

Hosted by Hills Creek Baptist Church

Please fill out the following form.

In consideration of being allowed to participate in any contests, games, or physical activities ("Activities") held during the Greenbrier Mountain Festival, I, the undersigned, on behalf of myself, my child(ren), or legal wards, acknowledge and agree to the following:

Voluntary Participation: I understand that my participation in the Activities is entirely voluntary and is for recreational and entertainment purposes. These Activities may include, but are not limited to, watermelon eating contests, apple bobbing, and other similar short-form competitions.
Assumption of Risk: I acknowledge that participation in these Activities carries with it certain inherent risks, including but not limited to choking hazards, allergic reactions, physical contact, slipping, falling, and other unforeseeable injuries. I assume all risks, known and unknown, associated with my participation.
Medical Fitness: I certify that I am physically able to participate in these Activities, and I am not aware of any health conditions or concerns that would disqualify me or my child from participating safely.
Release and Hold Harmless: I hereby waive, release, and discharge Hills Creek Baptist Church, the Greenbrier Mountain Festival, their respective pastors, officers, staff, volunteers, agents, and representatives from any and all liability, claims, demands, actions, or causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained during, or resulting from, participation in the Activities, whether caused by the negligence of the releasees or otherwise.
Indemnification: I agree to indemnify and hold harmless the released parties from any loss, liability, damage, or costs, including court costs and attorney's fees, that may arise due to my participation or the participation of my child.
Photo/Video Release: I grant permission for any photos or videos taken during the event to be used for promotional, social media, or documentation purposes by Hills Creek Baptist Church or the Greenbrier Mountain Festival without compensation or further consent.
Severability: If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
